When is flat feet normal and when does it require consulting a physical therapist? Free to Move Center Jeddah

Flat feet are very common in childhood. In some cases, they are normal and disappear with growth, but in other cases, they require follow-up and intervention. Here, we explain the differences, the warning signs, and how to examine your child at home.

What is flat foot?

Flat feet mean the arch of the foot is not visible and the entire sole of the foot touches the ground. In most children, the arch develops gradually until the age of 5–6 years.

When is it normal?

Under 5 years: Usually very normal and does not require intervention.
After 5 years: It is preferable to consult Physical Therapist Or a pediatric orthopedist to be sure.

When to worry?

  • Frequent pain or fatigue in the foot or leg.
  • Unsteady gait or frequent falls.
  • An arch that is not visible even when standing on tiptoes.
  • Flat feet persist after 6 years with symptoms.

Reasons

  • Genetic factors.
  • Weak ligaments and increased flexibility.
  • Nerve or muscle problems.
  • Inappropriate shoes for a long time.

Walker hazards

Walkers can cause poor posture, foot and leg deformities, and increase the risk of toe walking.

Home test

  1. Have the child stand barefoot on a level floor.
  2. Have him stand on tiptoes. If the arch appears, it's normal. If it doesn't, it needs an evaluation.
  3. Monitor your gait: Does he fall or complain of frequent pain?

Treatment

  • Supportive shoes.
  • Physical therapy exercises (heel/toe walking, picking things up with fingers).
  • Orthosis for painful or unstable conditions.
  • Botox 4-6 years in cases of severe muscle tension.
  • Surgery after 8 years in hard and severe cases.

Does every child with flat feet need treatment?

No, most children are completely normal. We only intervene if the pain or flat feet persist.

Do orthopedic shoes fix the problem?

They help reduce pain and improve gait, but they do not create a new arch on their own.
